Admissions and Supply Lists

Thank you for your interest in enrolling your child at Osceola Adventist Christian School. In order to start the enrollment process, you will need to complete the admission forms that may be accessed below.  The forms may be printed and must be filled out completely.  Return the forms to the OACS office before the school can consider the applicant for admission.

Please attach the application fee along with your child(ren)'s application. You can find fees on the Tuition and Donations link.  Once your child has been accepted, all forms listed below must be completed in full and turned into the school office before the child may start classes:

  4. Language and Education Information
  5. Annual Field Trip Permission Form
  6. Parent Contract
  7. Consent to Treatment
  8. Emergency Contact & Pick Up List
  9. Financial Agreement
  10. Internet Usage Agreement & BYOD Policy
  11. Media Consent and Release
  12. Request for Student Records
  13. Recommendation Form (To be completed by prior teacher and faxed to the school by the prior school)
  14. HRS Immunization Form 680 (Obtained from County Health Department)
  15. Physical Exam Form DH 3040 (New students, PK, K, 7th)
  16. Last Report Card (Obtained from your child's last school)
  17. Any Special Education (i.e. IEP, 504, Gifted and Talented) Forms (Obtained from last school attended)
